Before your first visit:

Once your appointment is scheduled please complete steps 1-5 by downloading the following forms in addition to completing the online history assessment. If the patient is under 18 please complete steps 1-6. If the patient is of Medicare age please complete steps 1-5 &7. You may return the completed downloaded forms by e-mail or bring them with you to your scheduled appointment. To return them via e-mail please send them to drcollins@broadstonechiro.com. What to expect on your first visit:

Consultation
You will have a one-on-one consultation and complete health history with Doctor Collins to discuss your health-related problems, concerns, as well as potential treatment options. Your first visit is designed for the Doctor to learn more about you, your condition, and your expectations to meet your health goals.  Dr. Collins takes pride in getting the whole story about not just your aches and pains, but about you as a person and what you want to be able to achieve in life.

Examination
After your consultation, Doctor Collins will perform a complete orthopedic, neurological, and chiropractic evaluation

Cancellation Policy for Initial Exam Appointments:

A credit card is required to be kept on file when you schedule your first appointment. Your credit card will be charged a fee of $50 IF you cancel or reschedule your initial appointment with less than 24 hours notice or if you do not show up for your initial appointment. It is your responsibility to ensure that we have your insurance information with enough time to verify your benefits before the cancellation window. It is also your responsibility to ensure that you can follow Broadstone Chiropractic’s mask policy as written below.

Mask Policy:

A mask covering the mouth and nose is required on all individuals over the age of 2 years old in order to receive services at Broadstone Chiropractic. Exemptions will not be accepted.
